Almost every rental car in Phuket is an automatic, and that is geography rather than fashion. The island is hills and stop-start traffic, and a manual turns both of those into work. People who want a manual here usually want it because they drive one at home.

Why the island drives automatics
Phuket has no long flat runs. The west coast road climbs and drops between every beach, and the passes between Kata, Karon and Patong are steep enough that a manual has you working the clutch on a hill start with a scooter tucked behind your bumper. Do that twice a day for a week and the novelty wears thin.
Patong in the evening
The second reason is traffic. Beach Road in the evening moves in metres. In an automatic that is boring. In a manual it is a workout for your left leg, in the heat, with the air conditioning on. Nobody has ever regretted taking the automatic in that queue.
There is also the driving-position issue. Cars here are right-hand drive, and if you learned on the left you are already relearning mirrors, roundabouts and turnings. Removing the gearstick from that list frees up attention for the road.
What it means for the car
An automatic in a hot climate lives or dies by its cooling. The gearbox and the air conditioning are both working hard, all day, on a car that spends a lot of its life crawling. This is why a car that shifts smoothly at handover matters more than the badge on the bonnet.
The gear limiting mode
Look at the selector for L, or a plus and minus, or an S. On a long descent, drop into it. The engine holds the car back and the brakes stay cool. Riding the brakes all the way down from the Kata viewpoint is how people arrive at the bottom with a soft pedal and a burning smell.
Climbing and the air conditioning
On a steep climb with a full car, the box will hold a lower gear and the engine will get loud. That is correct behaviour, not a fault. Let it do its job instead of feathering the throttle. If the air conditioning goes noticeably weak on the climb, mention it, because that is worth checking rather than living with.
Your first hours behind the wheel
The first drive should be a short one, and ideally not at night in the rain from the airport. Get out of the car park, do a lap of somewhere quiet, and let your hands learn where things are before the traffic gets a vote.
A checklist for the first few minutes
Everything below takes two minutes and removes most first-day surprises. The indicator stalk in particular catches people out: in a right-hand-drive car it is usually on the right, and the first few turnings come with the wipers on.
- Find the indicator stalk before you move, not at the first junction
- Locate L or S on the selector and try it once on a gentle slope
- Set the mirrors properly while stationary, including the left kerb view
- Check the air conditioning cools while the car is idling, not just moving
When people still take a manual
There are two honest cases. The first is a driver who genuinely prefers a manual, drives one daily and finds an automatic vague. That is a real preference and manuals do exist here, just in smaller numbers, so ask early.
The second case
The second is someone taking a pickup for work rather than for a holiday. Older working pickups are more often manual, and if the car is going to carry load on rough ground, that is a reasonable pairing.
Outside those two, the automatic wins. It is not about skill. It is about which of your hands and feet you want available when a scooter appears from the left on a blind bend.
Cars that suit this
For a couple staying near one beach, a compact automatic is ideal: light, easy to park in the narrow streets behind the beachfront, and cheap to run on short daily hops.
For hill roads with a full car, a crossover with an automatic gives you a bit more torque and a calmer climb. You will not need four-wheel drive anywhere on this island, but you will appreciate not having the engine at maximum revs every time the road goes up.
